If you are interested in becoming an individual member then fill in this form and send or fax it to the address below along with the appropriate amount of money to cover the membership fee.

It is understood that Umbrella expects all members to provide high quality professional care, and that its members share in the spirit of the non-profit purpose by involving the community in the operation of their centre.

Membership in Umbrella confers the right to one vote at Umbrella's Annual meeting, one free newsletter subscription and to unlimited use of services as listed in services. 

*Membership dues are payable annually on the following basis:

1-35 licensed full time child spaces       $ 90.00
34-60 licensed full-time child spaces    $150.00
61+ licenced full-time child spaces        $210.00

* Based on licensed capacity per year. Resource centres annual fee is $75.00.
